چکیده

Introduction Atrial fibrillation (AF) is the most common arrhythmia in humans. Its maintenance is thought to be due to multiple wavelets reentry occurring in both atria [1]. Previous studies have demonstrated the feasibility and efficacy of cryoablation to create lesions in both atria [2] and on the posterior wall of the left atrium [3], connecting the four pulmonary veins and the mitral annulus [4], in patients with chronic AF undergoing valvular heart surgery, as a valid alternative to the Maze procedure. Recently, the pulmonary veins (PVs) have been demonstrated to often play an important role in generating atrial fibrillation [5-7]. Because of their critical role in AF, a variety of surgical and catheter ablation techniques has been used to isolate the PVs from the left atrium [8-10].

New Insights On Ablation Of Persistent Atrial Fibrillation: Evidence From The SARA Trial.

Since Haissaguerre et al first described the pathogenic role of pulmonary vein firing as a crucial mechanism triggering atrial fibrillation, catheter ablation has been recommended as a curative treatment. Several trials have demonstrated that ablation is an effective treatment in most patients with paroxysmal atrial fibrillation and low-grade remodelled atria. In patients with persistent AF, th...

Single Center Experience with a Closed-Loop Irrigated Ablation Catheter for the Treatment of Human Paroxysmal and Persistent Atrial Fibrillation

Open-irrigated electrode catheters are commonly used for ablation for atrial fibrillation (AF). There are scant data available for closed-loop irrigation for ablation for AF. Proponents of open-irrigated catheters state greater patient safety from char formation, less impedance rises, and greater lesion depth. There are fundamental biophysical differences in the biophysics of the energy deliver...

Surgical and concomitant epicardial-endocardial (hybrid) ablation of persistent and long-standing persistent atrial fibrillation.

Catheter ablation of atrial fibrillation (AF) has been shown to be effective for paroxysmal AF. However, for patients with persistent or longstanding persistent AF, the success rates for catheter ablation is low. The Cox-Maze procedure is the most effective non-pharmacological treatment of AF. However, due to the need for open-heart surgery and the morbidity associated with the surgical Cox-Maz...

Catheter Ablation of Long Standing Persistent Atrial Fibrillation: Lessons Learned.

Atrial fibrillation has evolved from being a novel unproven procedure to being an important treatment option for patients with symptomatic atrial fibrillation. Atrial fibrillation ablation is an appropriate treatment option for patients with symptomatic atrial fibrillation, particularly if they have failed one or more trials of antiarrhythmic drug therapy. While much has been learned about the ...

Persistent Atrial Fibrillation Is Best Ablated by a Strategy That Terminates the Arrhythmia Procedural Termination Is Associated With Improved Long-Term Outcomes

Although effective results are attained with paroxysmal atrial fibrillation (AF) ablation based on pulmonary vein isolation (PVI), catheter ablation of persistent AF remains a challenging task. Current ablation approaches in persistent AF ablation are markedly diverse, and there is a considerable variation in the reported clinical outcomes. Moreover, the lack of a distinct procedural end point ...

Current Hot Potatoes in Atrial Fibrillation Ablation

Atrial fibrillation (AF) ablation has evolved to the treatment of choice for patients with drug-resistant and symptomatic AF. Pulmonary vein isolation at the ostial or antral level usually is sufficient for treatment of true paroxysmal AF. For persistent AF ablation, drivers and perpetuators outside of the pulmonary veins are responsible for AF maintenance and have to be targeted to achieve sat...
